Community Health Associates offers services to all individuals. A thorough clinical assessment identifies the person's strengths, needs, abilities and personal preferences. Our staff uses this information to formulate an individualized service plan (ISP) that guides the treatment process from beginning to end. Our philosophy of care is dedicated to helping to improve the quality of lives of children, adults, families and the communities they live in, by building upon strengths.

Our philosophy supports every customer's right to receive care in the least restrictive setting, to live independently when possible, to be employed, to lead a healthy lifestyle, and to be educated. Our staff is dedicated to helping consumers fully exercise their rights while learing how to use local resources and systems of care to meet individual and family needs.

Community Health Associates is licensed by the Arizona Department of Health Services & Office of Behavioral Health Licensure to provide outpatient behavioral health treatment and substance use services for children, youth, adults and families.

Community Health Associates, Inc., is a company dedicated to serving the mental health and physical health needs of children, adolescents, adults and families living in Yuma County, South County, La Paz County, Cochise County, Santa Cruz County, Pima County and Pinal County.
We are also a provider of Certified DUI Screening, Education, and Treatment services, and Certified Misdemeanor Offender Domestic Violence Counseling services.
CHA is a Certified AHCCCS provider and a contracted network provider for Arizona Complete Health Services of Arizona.

The CMT is dispatched by NurseWise, a team of nurses and behavioral health staff available to assist those experiencing a crisis and need immediate assistance, 24 hours a day, 365 days a year.
NurseWise can be called by family members, neighbors, friends, landlords, clergy or any other person(s) concerned about an individual.
NurseWise is a critical member of the Cenpatico Behavioral Health of Arizona.
The CMT responds to your community: homes, schools, Police/Fire Departments, churches/temples, medical offices.
