I am a preferred provider for Premera Blue Cross, Lifewise of WA, Group Health, Cigna, Regence, Molina, Amerigroup, United Behavioral Health/Optum Health, United Healthcare, CHPW, Aetna, and First Choice Health and can bill as an out of network provider for multiple insurances.
If you have a child in the foster care system or a child adopted who has an open Provider One coupon, I am also able to see them under medicaid.

My professional experience has included working with individuals and groups in a variety of environments including Young House Family Services, Lutheran Community Services, Excelsior Youth Center, Catholic Family and Child Service, and Smith Counseling Services, PLLC.
I also have volunteered as a camp counselor at Camp To Belong of WA, as a mentor through Ignite Youth Mentoring in Tri-Cities, and as a tutor/mentor through Extra Mile Student Center of Grandview.

If you are seeking services for your child and they are 12 or younger, please have the primary caregiver(s) fill out the documents about the child.
If the youth is 13-17, please have them fill out as much as they can and they will sign on the "Client/Individual" signature lines.
Please assist your child in completing the forms in totality.
